Registered Nurse Field Care Manager
Humana At Home (SeniorBridge) is seeking Registered Nurses to join our Care Management Network as an Independent Contractor.
SeniorBridge (SB) is a geriatric care management and home care company that helps people understand, manage and master the difficulties of aging.
We work with people who have complex and chronic medical conditions and functional deficits.
The company's unique and holistic approach addresses the total well-being of our clients and their families with the goal of keeping our clients safe and cared for in the comfort of their home.
Care Managers, both Nurses and Social Workers, provide a comprehensive approach of assessment planning service coordination and advocacy. Without the type of care offered by SeniorBridge, people with chronic conditions would be placed in nursing homes, dementia units or other type of long term care institutions.
SeniorBridge was founded in 1999 as a privately held corporation and was acquired by Humana in July of 2012.
We currently have 50 offices in 14 states. In addition, we have a Care Management Network of highly vetted, independent practitioners who are able to serve clients in areas where we do not have offices and to help us with the volume
of care management needed through the long-term and transitional programs that we manage for Humana Members.
