This position performs all duties in a manner that promotes team concepts and reflects the Providence Health & Services mission, philosophy, and core values of respect, compassion, justice, excellence and stewardship. In this position you will:
Ensures high quality patient outcomes and collaborates Executive Director, Clinical Directors and Clinical Managers in development and evaluation of the Quality Assessment Performance Improvement Plan annually (including the Safety Plan and Infection Prevention Plan). Collaborates with multi-disciplinary workgroups, using proven performance improvement, change acceleration and facilitation methodologies to ensure clinical and/or operational effectiveness and excellence, quality outcomes, patient safety, and positively impact patient/family satisfaction. Identifies areas for process improvement and facilitates efforts to address changes needed for improvement. Researches, formulates and implements best practice with Home Health Leadership and performance improvement teams. Serves as key expert on the Quality Committees and Safety Committees. Provides ongoing leadership and guidance for survey readiness to meet Regulatory Compliance. Conducts and instructs others in conducting root cause analysis and safety event reviews. Participates in the orientation, education, and coaching of clinical personnel as needed in coordination with the clinical education team. Monitor audit trends and formulate, interpret and implement practice changes as appropriate. Coordinates routine chart audits, plan of care and schedules focused audits as needed to further quality improvement efforts. Participate and review Home Health Policies in conjunction with ministry leadership. Ensures conformity of agency policies, procedures, performance and professional standards. Working with managers/supervisors to ensure compliance with regulations and accreditation standards.
Qualifications:
Required qualifications for this position include:
Bachelor's Degree in Nursing, Physical Therapy, Speech Therapy, Occupational Therapy 5 years clinical practice Current clinical license required in the state in which the caregiver works Possesses and maintains current CPR Certification Must have a valid Driver's License and maintain state required auto insurance Preferred qualification for this position include:
Master's Degree 2 years prior supervisory experience 1 year Home Health based experience 1 year experience with accreditation regulations as well as State regulations 1 year project management experience
Salary Range:
$100K -- $150K
Minimum Qualification
Nursing, Healthcare ManagementEstimated Salary: $20 to $28 per hour based on qualifications.
